como hacer Aplicacion web adaptable

Si subo la aplicación creada a un servidor, como le hago para que dependiendo de que aparato estén accediendo a la pagina se vea correctamente en el mismo?
ejemplo, podria crear una para smartphones, otra para tablets y otra por si se accede desde una pc
Pero existe algun plugin o codigo para que se detecte el tipo de conexion y envie a la url con la aplicacion correcta ?
